Patient Navigator

ResponsibilitiesMaintain accurate files, calendars, schedules, contact lists, ad dashboards
Book air travel through UCLA's travel center for patients and guests
Coordinate all ground transportation for patients and guests
Coordinate hotel schedules at the Tiverton House Hotel
Track all travel and assist with data collection
Provides age appropriate assessment and interventions in order to meet the needs of patients and families served
Accompany UCLA Patient Escort services and Operation Mend drivers to and from airports, medical offices, and residences on and off of UCLA's Westwood and Santa Monica campus as needed
Escort patients and family members to designated locations and often times wait for designated appointments, surgeries, or events to be complete
Proactively build relationships with patients and family members and anticipate special needs
Work in conjunction with Operation Mend's team to optimize the patient and family experience through their stay with Operation Mend and when they return home
Coordinate all accommodations for Operation Mend guests (military leaders, academic medical center visitors, etc) including hotels, transportation, restaurant reservations, etc
Facilitate prompt and direct communication with case managers and other care providers outside of UCLA as it relates to patient itineraries, per diem, proffer letters, etc.
Assist with schedule coordination for volunteers and interns as it pertains to patient transportation and escorting
Actively update Operation Mend team on all updates related to patient coordination and act as their liaison with unexpected changes to travel, etc.
Problem solve under pressure and evaluate solutions based on what is best for individual patients, family members, staff members, and the Operation Mend team
Support social media and photo gathering for various Operation Mend platforms
Add appointments, surgeries and flight information to VisionTree once scheduled
Maintain strong team communication as it pertains to patient care
Assist with multiple patients and their families in both a clinical setting and at the airport

QualificationsExperience working with military patients and families is desired.
Experience working with patients with varying illnesses, injuries, and disabilities is desired.
Bachelor's degree preferred.
Skill in exercising good judgment in referring calls to faculty.
Working knowledge of Travel Express, Connexus, and Bruinbuy is strongly desired.
Working knowledge of departmental policies and procedures on the safe transport of patients.
Knowledge of the UCLA Health landscape and medical campuses is strongly preferred.
Ability to establish and maintain cooperative working relationship with other staff members and all other personnel.
Skill in listening and comprehending information received in person or over the telephone to navigate patients.
Ability to anticipate specific needs for individual patients, family members, and staff.
Detail oriented and ability to keep accurate and thorough records.
Ability to work standing, walking, and sitting for extended periods of time.
Interpersonal skills to interact diplomatically and sensitively with patients, staff, and visitors.
Working knowledge of Microsoft Office Suite programs (excel, word, PowerPoint, etc.) is desired.
Knowledge of CareConnect and Visiontree preferred.
Experience working with social media is desired.
Ability to develop priorities, meet deadlines despite interruptions, and perform multiple complex assignments.
Ability to work independently on assignments with minimal supervision.
Must have a valid CA DMV Driver's license/Recent DMV Record



Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
