Clinical Social Worker, Per Diem - Wknds, ER/Psych

Job Description:
Responsibilities Provide in- and outpatient psychosocial transplant evaluations, therapeutic interventions, and community resource support information and referrals for patients and their families.
Complete written assessment documents in a timely way for use by treatment team.
Collaborate closely with other social work and interdisciplinary colleagues, including residents, fellows, and transplant coordinators, as well as social workers at other hospitals.
Develop and implement appropriate psychosocial interventions that enhance patient and family preparation for and adherence to medical, surgical, and pharmacological regimens.
Teach families and hospital teams about diagnosis-specific community resources and insurances.
Participate actively in interdisciplinary team rounds, clinics, selection committee, and other committees.
Participate actively in provision of patient/family education.
Organize and facilitate support group(s) when indicated.
Participate in case coverage and other necessary support for other social workers in the Dept.
of Care Coordination.
Maintain accurate statistical records.
When appropriate, collect and provide data for program development, research, and publication.
Participate in formal and informal teaching, including supervision of social work graduate students and in-service education for nurses, nursing students, medical residents, fellows, interns, and ancillary staff.
Qualifications Master's Degree in Social Work (MSW) from a program accredited by the United States Council on Social Work Education required.
CA LCSW preferred OR CA License eligible.
At least two years post-Master's experience with medical patients preferred.
Understanding of emotional reactions to serious/catastrophic diagnoses / illness / and aging.
Demonstrated skill in clinical assessment and treatment, crisis intervention, end-of-life issues, and bereavement.
Demonstrated experience as a member of an interdisciplinary team.
Knowledge about insurances (especially Medicare, Medi-Cal).
Knowledge about substance abuse assessment and treatment practices, protocols, and resources.
Awareness of other relevant community resources, especially for transportation, counseling, and financial aid.
Group work experience helpful.
Well-developed interpersonal skills and customer service orientation.
Understanding of cultural differences and racial and religious diversity.
Knowledge of Child Abuse / Elder & Dependent Adult Abuse / Neglect and Domestic Violence treatment interventions and reporting protocols.
Commitment to principles of patient- and family-centered care.
Experience with principles of bioethics.
Ability to handle fast-paced clinical work environment.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
